Why Evaluation or Assessment?

An evaluation helps to make a determination as to the level of care needed to properly address client need. Some clients must get a DRUG & ALCOHOL evaluation to fulfill a COURT REQUIREMENT. An evaluation is typically the starting point of the process.

Drug & Alcohol Evaluation


A Drug and Alcohol Assessment usually takes less than one hour to complete and is done by appointment only. The client and a Capstone staff member go through basic information, the information is then considered and a recommendation given.

Domestic Violence Evaluation (currently not offered through Capstone)


Useful information: This evaluation is a bit more extensive and the client is required to provide the following information before the recommendation can be determined:

  • BCI Background Screening; which can be obtained in person or by mail (contact Capstone for details, or follow the following link for more details: http://bci.utah.gov/criminal-records/obtain-a-copy-of-your-own-criminal-history-record/)
  • Police Report; which can be obtained from the police department, or from your attorney

The client's account of the situation, the BCI & Police Report, along with the victim's report of the events are considered as a whole, and a recommendation is given only after consideration of all of these accounts.

Mental Health Evaluation


This evaluation is conducted by a Capstone therapist, typically at the request of an intervening agency, to determine underlying issues to behaviors.
